Initial applications for Teacher Certification are now completed through the Teacher Information Management System (TIMS) portal on the Pennsylvania Department of Education website:

  Please download, complete, and mail the above document to Dr. Phyllis Miller, Education Certification Coordinator, Room 104 Upland Center, 540 Upland Avenue, Reading, PA 19611.  Completion of the form will assist the Education Office in tracking the certification applications filed by students.
