Additional Health Requirements


Experiential learning is at the heart of Alvernia University’s academic programs. Students have the opportunity to function in environments and agencies that serve many people in our region. Experts in the field are available to students as the classroom and practice settings come together.

There are many laws, policies, and regulations that govern agencies providing services to vulnerable populations and the general public. In addition to meeting academic program requirements, students need to be of good health and character to participate in these opportunities.

Legal contracts (affiliation agreements) have been developed in partnership with agencies that allow our students to work with a variety of clients, patients, and children. Alvernia University is committed to uphold the standards in our agreements.

The following tables describe common requirements outlined in Alvernia’s agency affiliation agreements. Please note that additional mandates may be required with individual agencies. Work with your academic department field coordinator or secretary to verify agency requirements:

  Medical History   Completed Alvernia Medical History within one year of starting academic program at Alvernia.

  Physical Exam   Written documentation from healthcare provider of physical exam performed within one year of starting academic program at Alvernia. Some programs or off-campus internship sites require an annual physical exam.  

Mail or fax to Health and Wellness Center or upload form on the Alvernia Student Health Portal.

  Tuberculosis (TB) Testing   Two negative skin tests; one within the past year or tuberculosis blood test or proof of treatment for latent tuberculosis.  

Tuberculosis skin tests are available by appointment at the Health and Wellness Center for a small fee.

If tuberculosis testing is completed off-site, record of the testing should be submitted by mail or fax to Health and Wellness Center or uploaded on the Alvernia Student Health Portal.

 
             
             
             
  Measles, Mumps, Rubella (MMR)   Proof of immunity against measles, mumps, and rubella. 2 MMR vaccinations or blood titers are required (submit lab report).

Vaccine may be available at Health & Wellness Center.  Call to inquire.

 
             
             
             
  Hepatitis B   3 Hepatitis B vaccinations or signed declination or Hepatits B surface antibody titer blood test (submit lab report).  

Mail or fax to Health and Wellness Center or upload on the Alvernia Student Health Portal.

  Varicella Vaccine   2 varicella vaccinations or a varicella titer documenting immunity.  

Mail or fax to Health and Wellness Center or upload on the Alvernia Student Health Portal.

  Tetanus/ Tetanus, Diphtheria, Pertussis (Tdap) Vaccine   Proof of at least one Tdap booster vaccination at or after age 11 and a TD if Tdap was given greater than 10 years ago.

  Influenza Vaccine   Current seasonal influenza vaccination (given after 8/1 of current season).  

Influenza vaccines are available by appointment at the Health and Wellness Center for a small fee.

  Pneumovax Vaccine   Proof of immunization or signed declination.

  Drug Screen  

10-panel urine drug screen will be performed according to national and SJMC standards (includes marijuana, opiates, cocaine, amphetamines, PCP, barbiturates, benzodiazepine, methaqualone, propoxyphene, methadone).
Please refer to academic department policy for “positive results” procedure or consult the Health and Wellness Center.

 

Penn State Health St. Joseph is the designated laboratory. Testing locations are listed on the Referral Form.

Obtain the Referral Form from your academic department secretary.

Referral Form and photo ID are required for testing.

Each student will sign a release to allow Penn State Health St. Joseph to report results directly to the Health and Wellness Center.
